Disclosure

While all disclosures are confidential and can be made anonymously, you may choose to provide your name, phone number or email address. We will only contact you if we need more information. For any disclosure made, only the duly authorized staff members of the Direction de la conformité et des enquêtes of the Ministère de l'Éducation will have access to your personal information. If the disclosure concerns a teacher, it will be the duly authorized staff members of the Direction de l’encadrement du personnel enseignant of the Ministère de l'Éducation who will have access to the information. We assure you that under no circumstances will your personal information be shared or released without your consent.

As public bodies, the Ministères are subject to the Act respecting Access to documents held by public bodies and the Protection of personal information and will protect your identity in accordance with the provisions of this Act. Consequently, the Ministères will be unable to update you on the follow-up and results of their analysis.

However, under the Act respecting Access to documents held by public bodies and the Protection of personal information, you have the right to consult information that you have provided and to ask for the correction of any inaccurate, incomplete or ambiguous information, provided this information can be traced.

Please note that if you are a staff member of the the Ministère de l'Éducation or the Ministère de l'Enseignement supérieur and would like to gain access to the protection provided for in the Act to facilitate the disclosure of wrongdoings relating to public bodies, you can contact the Protecteur du citoyen, the only body authorized to receive disclosures from staff members of Québec’s public service.

Disclosure concerning a teacher

If you want to make a disclosure concerning a teacher who, in carrying out their duties, has behaved in such a way as to make one reasonably fear for the physical and psychological safety of students, please submit all the information you have that will help identify this person (full name, age, date of birth, address, name of the educational organization where they work, etc.). Please also make sure that you check off the "Teacher" option in Section 3 of the form and describe the allegations as precisely as possible by including the location(s) and time(s) they occurred.
